If you would like to have a copy of your Physician, Hospital, or Pharmacy bill/records sent to you please submit an online request. Please indicate the dates of services you are requesting in the message section of the online form. Pay your hospital bill online. Physician bills are currently not accepted online. You will need to have your paper statement available when paying online. Please include your account number on the Patient ID line of the online bill pay system.
