Community Care Financial Assistance Program
If you or your family is facing financial difficulties, please take a moment to talk with the Hopedale Medical Complex staff to learn more about Community Care Financial Assistance.
As a not-for-profit charitable corporation, the Hopedale Medical Foundation does not deny services based on one’s inability to pay.
Community Care is Hopedale Medical Complex’s financial assistance program which offers our patients and residents discounted (or entirely free) services.
It is available to those who apply and who qualify based principally upon the individual’s or family’s gross income and available assets.
Community Care is for persons who do not qualify for other assistance programs such as Public Aid and for those who are not already receiving the same or similar services elsewhere.
If you qualify for financial assistance, you will be notified as soon as possible regarding the amount of the discount and a payment plan (if the discount is less than 100%).
HMC reserves the right to reconsider approval if we receive information that a family’s financial situation has changed or false information was provided or omitted.
No person can be denied access to Emergency Services for financial reasons.
How to Apply
To receive Community Care, you (or your guardian/Power of Attorney) must fill out an application and submit it with the requested paperwork and financial documentation to:

Notice: Community Care does not apply to private physicians’ services. These physicians are independent contractors. If you need assistance with paying your physician’s bill, please contact their offices directly.
