What is the difference between Independent Living, Assisted Living and Skilled Nursing Facility?
Independent Living at Commons East includes the basic services of three homestyle meals seven days a week, all utilities, cable, transportation to appointments on HMC campus, activities, and a night time manager. The resident leads an independent lifestyle which requires no extra assistance.
Assisted Living at Hopedale Commons West provides, 40 personalized services designed to meet the needs of those who require help with activities of daily living.
A Nursing Home, also known as a Skilled Nursing Facility, has licensed Nurses who provide 24-hour care to residents who can no longer care for themselves due to physical, emotional or mental conditions.

Hopedale Commons is located in Hopedale ,IL. 30 miles south of Peoria, Il., 30 miles west of Bloomington, Il., 50 miles north of Springfield, Il. See map.
What is the benefit of residing at Hopedale Commons East, West or the Nursing Home?
The unique and most attractive benefit of residing at Hopedale Commons or NH is access to our very efficient and comprehensive healthcare complex; all of our communities are located on one campus. Our experienced physicians care for our residents including any ER care or surgeries performed in our acute care hospital.

Assisted Living FAQs: Commons West
What services are provided in Assisted Living?
Residents of Commons West Assisted Living receive 40 personalized services, 3 meals a day, daily housekeeping service, personal laundry services, weekly bed linen services, daily bed-making services, complimentary transportation services, mail delivery, paid utilities, nightly room checks, apartment maintenance and repair, and access to on-site wellness programs & daily planned activities. Additional services such as assistance with activities of daily living ,ie, assist with bathing, dressing, and medication set-up, and medication reminders are also included in our all-inclusive monthly rates.
How big are the apartments?
Commons West offers 2 basic floor plans each are 660 sq. ft. Monthly rental fees are for 1-bedroom apartments. With an additional fee if apartment is furnished.
Where are the meals served?
Residents of Commons West Assisted Living enjoy their daily meals in the Great Room Dining area, situated in the center of the building. The dining room offers the same menu as Independent Living with table-service dining. Hopedale Commons as part of the Hopedale Medical Complex, prides itself in offering healthy food choices including homemade bread and homemade desserts daily.
How is the level of service determined?
A Pre ‘move- in’ medical evaluation (physical) is scheduled for each potential resident to determine his/her healthcare needs and thus their level of care ,ie, independent, assisted or nursing home.
Information provided from the physician’s assessment, family assessment and Assisted Living Director’s assessment are combined to determine the level of care best suited for each resident. A resident’s level of service can fluctuate based on an individual’s increased or decreased needs and will be re-addressed by the Assisted Living Director as appropriate.
Can residents in Assisted Living attend activities/social events in other parts of the HMC campus?
Certainly!  Residents of Commons West are encouraged to take advantage of all activities and social events offered. Monthly activity calendars are posted and distributed to each resident. Residents are encouraged each day to participate in “sittercise†which is an in-house activity with current events and does not require freestanding physical movement.  There are several entertainment events hosted in the Grand Room of Commons East our neighbor next door to Commons West. Our staff is very accommodating and assists residents with walking or transporting  to these events.
What payment options are available in Assisted Living?
Residents or their Power of Attorneys will be billed on a monthly basis. Payments may be made by check, direct bank debit, credit card. For tax purposes, in state of Illinois, assisted living payments are tax deductible with proof of healthcare services being provided. Residents in Assisted Living enter into a rental agreement with HMC. Residents will receive a monthly statement highlighting their billing charges.

Is financial support available?
Hopedale Medical Foundation is a not-for- profit corporation which funds a charitable assistance program to subsidize monthly rental payments for Commons residents and nursing home residents. Residents must complete an application and be qualified.. Each application is ruled on and determined on an individual basis according to need.
When should interested individuals apply for residency?
Families of residents should first call the Director do discuss the services and monthly rate of the Commons or nursing home. Then, if they realize they can not meet the monthly rate, they should apply for financial assistance.
What activities are offered?

Independent Living FAQs: Commons East
Is my apartment furnished?
Apartments may be furnished upon request. There is a monthly fee for this service. Some residents want to try out a furnished apartment for the first month and then move their own furniture in later. Commons provides residents and families with moving assistance at no charge. Microwave, small refrigerator, telephone are furnished at no charge with each apartment.
How many floor plans are available?
Commons East features Independent Living one-bedroom apartments that are 660 sq. ft.
May I have overnight guests?
Yes, guests are certainly welcome to stay overnight in resident’s apartment, with resident’s approval. If guests would prefer to have their own room, guest apartments are available for a nominal fee.
When can I move into my apartment?
Apartments may be chosen and reserved for 60 days by paying a security deposit to hold the apartment. If resident decides not to take the apartment and notifies the Director with in 2 weeks, 50% of the deposit will be returned. Otherwise, a resident may move into his/her apartment as soon as the contract is completed and is signed by resident and Power of Attorney (POA). Before setting a move-in date, the individual will need to complete and return all necessary move-in paperwork along with completed physician’s forms. On the designated move-in date, the individual will sign a resident contract, begin paying monthly fees and receive keys to their new apartment.
What do I do if I have a maintenance issue?
If you are having problems with plumbing, heating/cooling, appliances in your apartment or you have any other maintenance issue, simply call the ‘HELP’ number located on your telephone , complete a maintenance work order found on table in common area or inform a staff member of the problem and it will be reported to the Director.

Is financial support available?
Residents who need financial assistance may complete an application for community care which is Hopedale Medical Foundation’s funding for subsidized rental assistance. This application will then go to committee and be reviewed on basis of individual need.

Skilled Nursing Facility FAQs: Hopedale Nursing Home
Do you have to be a resident of Commons East or Commons West to be admitted to Hopdale Nursing Home?
No, admission is open to individuals from the community as well as residents of Commons East: Assisted Living and Commons West: Independent Living.
Is Hopedale Nursing Home Medicare-licensed?
The Hopedale Medical Complex accepts Medicare/Medicaid patients who need rehabilitative extended care into our ‘swing beds’ in the hospital. When a person plateaus or ‘rehabilitates to their baseline’, they may be discharged to an alternate level of care. If the person is in need of continued long term care, they may consider the nursing home for continued rehabilitation or maintenance therapy. Hopedale Nursing Home stays are not covered by Medicare or Medicaid. In some instances, private insurance or long term care insurance will reimburse residents for their nursing home care. It is always smart to verify coverage with your insurance company prior to seeking admission.
Are private rooms available?
The Hopedale Nursing Home offers both private and semi private nursing home rooms. Our spacious rooms permit residents to bring in private items from home to give their rooms that cozy ‘homelike’ atmosphere.
How much clothing should I bring?
We recommend at least seven complete outfits. Appropriate clothing is usually considered to be slacks, shirts or tops, dresses, sweatsuits, sweater, nightgown/sleepwear, and underwear. Shoes or slippers should have nonslip soles. As a general rule, clothing should be roomy and have large enough openings that they can be put on and removed with a minimum of stress to both the resident and the clothing.
- Personal laundry service is provided by the facility at no charge, however, you may choose to forgo this service.
- Clothing should be machine washable and dryable.
- Each item of clothing should be indelibly marked with the resident’s name (first initial and last name).
- Families should expect to assist in maintaining the resident’s wardrobe.
